MEDICAL OFFICE VISITS

For new & follow up patients. This is a dedicated visit only for a skin cancer screening.
The discussion of additional concerns will increase the visit to a skin cancer screening PLUS visit (see below).

For new & follow up patients.
This is a dedicated visit only for hair loss consultation.

This visit is for a new patient to discuss ONE concern (example: acne consultation).
If more than one concern is discussed, the visit and pricing will be adjusted accordingly.

Does not include skin cancer screening or hair loss consultation which must be booked separately as dedicated visits.
This visit is for a new patient who wants to discuss up to 2-3 dermatologic concerns (example: consultation for acne & melasma in the same visit).

This visit is for a new patient who would like to book a comprehensive visit that includes a skin cancer screening PLUS evaluation of 1 dermatologic concern (example: a skin cancer screening visit and acne consultation).
Please contact the office for a quote and/or for pricing for a more comprehensive visit (skin cancer screening plus evaluation of multiple concerns).

This appointment is for follow up of a SINGLE issue, that has already been discussed at a prior visit (example: follow up of acne requiring refills).
If more than one issue or a new issue is discussed, the visit and pricing will be adjusted accordingly. -
This visit is for follow up of up to 2-3 issues that have already been discussed at a prior visit (example: follow up of acne and melasma) or for a follow up patient that is coming for a new evaluation of an issue (example: new acne consultation).
This visit type may also be required for an established patient needing to initiate systemic therapy, at the discretion of the office.

This visit for a follow up patient who is coming in for skin cancer screening PLUS follow up of already evaluated medical issues (example: follow up of acne requiring refills) OR skin cancer screening PLUS evaluation of a new issue (example: new evaluation of melasma).
COSMETIC TREATMENTS
○ A cosmetic consult is required before all cosmetic treatments to determine if you are an appropriate candidate for a procedure.
○ PRP injections for hair require an initial medical hair loss consultation.

Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
○ You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
○ Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
○ If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
○ Make sure to save a copy or picture of your Good Faith Estimate.
